Job Summary:

Contributes to the Tribe's mission of building and enhancing a Tribal Community rich in healthy families and capable people with strong cultural values by providing transport services to elderly Tribal members, which affords the opportunity for them to gather with peers for meals and social activities. Attends medical appointments supporting adherence to care and treatment, and empowering individuals to be agents in improving their health and extending access to health services in the tribal community.

Essential Functions:

  • Safely transports Tribal members to and from medical appointments.
  • Provides assistance to senior Tribal members while transporting them, such as safe boarding and unloading from vehicles, including disabled and wheelchair-bound persons.
  • Provide other transportation services to senior Tribal members as requested.
  • Maintains proper fluid levels in vehicles used, and reports any mechanical problems to the proper person.
  • Applies the Privacy Act as it impacts clients.
  • Assists with the setup and maintenance of the Medical Alert monitoring system, providing ongoing support to clients. Assists with the coordination of Farm Share.
  • Participates in department-sponsored activities that promote health and wellness.
  • Provide home safety visits for Tribal members.
  • Assists with non-medical transports and assigned activities.
  • Engages with Tribal members and community on a daily basis to promote, encourage and support healthy lifestyle behaviors.
  • Maintains accurate and current client records and program reporting as required.
  • Assists clients with appointment setting/care coordination.
  • Provides medication delivery to Tribal Members.
  • Provides basic medication management as needed for Tribal members.
  • Completes Wellness Visits as needed for Tribal members.
  • Participates in community outreach education events.
  • Acts as a responder as needed to natural or man-made disaster(s), such as incidents of multiple casualties.
  • Performs a variety of other duties as assigned by supervisor.

Education, Experience, and Licenses:

  • Requires a high school diploma or GED.
  • One (1) year of related work experience preferred.
  • OR equivalent combination of education, training and experience.

Driving Requirements:

  • Must maintain a valid Oregon driver's license or obtain one upon hire, qualify for the Tribe's vehicle insurance, and provide proof of personal vehicle insurance.

Qualifications, Registrations, and Certifications:

  • Requires First Aid and CPR certificate within the first 30 days of hire.
  • Must have a working knowledge of the Privacy Act and its application.
  • Knowledge of and sensitivity to Native American culture is required. Native American employees preferred.
  • Willing to develop new skills under the leadership of the department supervisor.

Environmental Factors and Conditions/Physical Requirements:

  • Continually requires expressing or receiving detailed information or important instructions through oral communication.
  • Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks.
  • Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
  • Occasionally requires lifting of up to 40 pounds.
  • Requires First Aid and CPR certificate
  • Must have a working knowledge of the Privacy Act and its application.
  • Continually involves sedentary work and sitting for sustained periods of time to operate passenger van and/or other vehicles.
  • Continually requires the ability to identify colors. Close and far vision and depth perception are required on a continual basis. Occasionally requires night vision.
  • Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.
  • This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.
  • May be called upon to serve as a Disaster Reservist during emergencies.
  • Local Travel is daily. Travel outside of the state as well as statewide for overnight trips occasionally.
