Job Summary:

Provides case management and coordination of clinical care services in an opioid treatment program (OTP).  Ensures clients receive appropriate treatment in a reasonable amount of time, with an individualized care plan. Contributes to accomplishment of the Tribe's holistic health and wellness   

 Essential Functions:

  • Responsible for ensuring the clinical care services are operating collectively to create whole person-centered care.
  • Facilitates case management services for clients.
  • Ensures clients receive timely, high quality and efficient health care and support services through the referrals to both internal and external resources.
  • Ensures culturally specific practices are followed with sensitivity to the AI/AN population's unique needs.
  • Reengages clients who have dropped out of care services.
  • Sends letters with resources and follow up information to all clients.
  • Navigates between medical and behavioral health referrals and various needs of  clients including maternity care.
  • Track's  client's ER visits and follow-up care.
  • Identifies and utilizes all appropriate available community resources.
  • Serves in a supporting role for both the clinical and medical teams.  
  • Collaborates with a multidisciplinary team in a recovery focused environment to reduce harmful effects of substance use and increase overall wellbeing of client's life and the communities in which they reside.
  • Responsible for ensuring a trauma focused work environment; including the ability to demonstrate an understanding of the historic implications of intergenerational trauma with regards to the AI/AN population.
  • Performs trauma informed care in a chaotic environment or a crisis.
  • Ensures that accurate information about clinic process, improvement, and challenges are reported to the executive director of Health and Wellness programs.
  • Networks with community agencies, health, housing, and education programs and with outside agencies and organizations to help client's access services and to help enhance or improve services.
  • Attends interdepartmental and interagency meetings.
  • Performs a variety of other duties as assigned by the Supervisor.

Education, Experience, and Licenses:

  • Requires a High School Diploma or GED; an Associates degree in Human Services or closely related field of study is preferred.
  • Requires two (2) years of work experience in case management services in the following areas: collaboration with mental health, addictions counselors, intellectual and/or developmental disabilities.  Experience in a substance use environment strongly preferred.
  • Certified Recovery Mentor or CADC certification required.
  • OR, equivalent combination of education, training and experience.

Driving Requirements:

  • Must maintain a valid Oregon driver's license or obtain one upon hire, qualify for the Tribe's vehicle insurance, and provide proof of personal vehicle insurance.

Qualifications, Registrations, and Certifications:

  • Requires two (2) years abstinence from alcohol and chemical dependency (if recovering person).
  • Adherences to the organization's policies and procedures covering the privacy, access, and client health information in compliance with Federal and state laws and the healthcare organization's information privacy practices. 
  • Must have a working knowledge of the Privacy Act/HIPPA and its applications.
  • CPR /First Aid certified required or ability to obtain certification within 90 days of hire. Must maintain throughout employment.
  • Must have strong interpersonal skills working with individuals from diverse backgrounds.
  • Must respect all others regardless of age, gender, ethnicity, or income level.
  • Must possess a strong knowledge base of medications to aid in the recovery process.
  • Requires working knowledge and proficiency in use of office equipment and computer software such as Microsoft Products.
  • Must have the ability to resolve conflict while utilizing conflict management philosophies.
  • Requires excellent verbal and written communication skills, business and telephone etiquette.
  • Must have the ability to work with minimal supervision.
  • Must have excellent organizational skills, attention to detail and record keeping skills.
  • Must have ability to respond courteously and tactfully to complaints and client requests.
  • Electronic Health Record (EHR) knowledge is highly desirable. Preferably with NextGen or Methasoft.
  • Knowledge of and sensitivity to Native American culture required.
  • During emergency operations may be required to report to work or remain at work.

Environmental Factors and Conditions/Physical Requirements:

  • Continually requires the ability to express or receive detailed information or important instructions through oral communication.
  • Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers.  Often requires walking or moving about to accomplish tasks.
  • Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
  • Occasionally requires lifting of up to 40 pounds.
  • Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.
  • This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.
  • Supports a safe work environment by performing all duties in accordance with Tribal safety policies and procedures, and Emergency Management Plan. 
  • May be called upon to serve as a Disaster Reservist during Emergencies.
  • Local travel and is occasionally required. Travel outside of the state is occasionally required.
